Use this form to change your address for an existing library card account. To change your phone number, your e-mail address, or sign up for text-message alerts for hold notices, log in to your account.
- Once you submit this form, you won't be able to renew items or place holds until a photo ID with current address is presented to library staff on your next visit.
- If the information submitted doesn't match the existing record, change(s) will not be made.
- Name changes must be requested in person at a Library location.
