Use this form to change your address for an existing library card account. To change your phone number, your e-mail address, or sign up for text-message alerts for hold notices, log in to your account.

Once you submit this form, you won't be able to renew items or place holds until a photo ID with current address is presented to library staff on your next visit.
If the information submitted doesn't match the existing record, change(s) will not be made.
Name changes must be requested in person at a Library location.
